A worker was painting the inside of the water tower. He was standing on a ladder 40 feet above the floor without wearing any fall protection equipment. Obviously he fell through an opening in the floor and died on the spot.
INSPECTION RESULTS
Following its inspection. Occupational Safety and Health Administration(管理局)listed three violations(违规)of its construction standards. Had the required fall protection been worn by the employee, his death could have been prevented.
ACCIDENT PREVENTION RECOMMENDATIONS
